FORKLIFT TRAINING COURSE: MANDURAH, WA

If you are interested in becoming a fully nationally qualified forklift driver, then you have come to the right place.

Here's some information about the course once enrolled:

Student must bring Valid Australian Identification.
Student must be 18 years or older.
There is a Worksafe fee of $90.00 payable to Worksafe once completed the assessments.
There is a Theory Exam and Practical demonstration Exam
Must be Australian Citizen or Permanent Resident.

Worksafe Requirements:

To apply for a High Risk Work Licence you will need to provide Worksafe WA:

A completed application form.
Notice of satisfactory assessment – white copy from your High Risk Trainer
Statement of Attainment from the RTO
Proof of identity – 1 form of ID Including this certification.
A head and shoulders photograph taken within the last six months x2 photos
Application fee

The following fees are current as of 1 July 2024.

Please note: No GST is payable on fees.

Application for licence (new issue, variation or interstate) $90
Application for Add a class to the licence: $45.10
Application for renewal of licence: $44.00
Application for replacement of lost or stolen licence: $29.00
